[Guides] Feedback form suggestions needed.

I think that if someone is upset/interested/motivated enough to provide
feedback at all, they won't mind typing out their thoughts in a free text
format. And even if it does stop some, better to have fewer people
provide more detailed feedback than to have an abundance of muddy or
limited feedback.
I think that there should also be a field for them to describe in some way
who they are/why they are reading the Guides so that we can get a sense of
our audience.
